The lawsuit brings claims against ConnectOnCall.com and Phreesia over the incident. The proposed settlement creates a $4.95 million fund. You can claim two years of CyEx Medical Shield Complete monitoring and either up to $5,000 for documented losses or an alternate cash payment of up to $75.
