Henderson & Walton Women's Center, P.C. (HWWC) has agreed to provide up to three years of free credit and medical identity monitoring and cash payments in response to allegations related to a cybersecurity breach that occurred between February 11, 2022, and February 14, 2022.
The data breach potentially compromised the personal and protected health information of approximately 34,000 current and former patients. Eligible class members can claim up to 3 years of free credit and medical identity monitoring, up to $150 for ordinary losses, up to $2,500 for extraordinary losses, and up to $90 for lost time.
Eligibility
You may be eligible if you are a person whose personal information and/or protected health information was potentially compromised as a result of the Cybersecurity Incident which occurred between February 11, 2022, and February 14, 2022.
