Eligible Legacy Health patients can receive cash payments of $15 and one year of free credit monitoring services through CyEx Medical Shield Complete creditor monitoring.
You may qualify for benefits if you are a U.S. resident and patient of Legacy Health who registered for the patient portal from February 18, 2019 to December 31, 2020. Even if you did not register during that period, but did access the patient portal between January 1, 2021 and February 9, 2024, you may be eligible.
